Self referrals
There are a number of services locally that you are able to make self referrals to.
GP referrals
To request a GP referral:
Provide as much information as you can as this will help us to process your request.
Tracking a referral
Once your referral has been processed you will receive correspondence from our secretary team. Please ensure you read the leaflet fully and follow the guidance in the leaflet referral information (PDF).

NHS referrals
Your referral letter will be typed and electronically submitted by the eRs (Electronic Referral System) and referral paperwork to you via Accurx. If we have been able to arrange an appointment for you, your appointment details will be in this paperwork, if you need to arrange your own appointment, then the booking reference number and access code are in this paperwork. If we have been unable to make an appointment for you, your paperwork will have your booking reference number and access code for you to make your own appointment.

Private referrals
- Please telephone your insurance company to check that your policy covers you for your request.
- You can either telephone the hospital of your choice to make your outpatient appointment or request an ‘open referral’
- Please email the secretaries’ office larksfield@nhs.net with the date and time of your appointment, along with the name of your consultant or advise if you require an ‘open referral’
- Your named consultant referral letter will be typed and sent electronically to the hospital of your choice or your open referral letter will be typed and sent to you via Accurx.
