Patient Participation Group registration

Use this service to join our Patient Participation Group to receive newsletters and invitations to contribute to the group.

You can use this service if you:

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

Your information will be stored securely by the practice and should you wish your details be taken off the PPG distribution list you can email ppg.larksfield@nhs.net at any time to request this.
